Specialist Support
Our Specialist Support service provides high-level advocacy for individuals with complex support needs. We collaborate closely with participants, families, carers, service providers, and government agencies to integrate supports effectively and align them with the participant's personal goals. Experienced Specialist Support Coordinators identify and address barriers to service access, stability, and positive outcomes, while developing practical strategies for risk management, crisis navigation, and building sustainable support networks that foster independence and long-term wellbeing. We are committed to empowering participants to make informed decisions, exercise choice and control, and maximise the value of their NDIS funding through ongoing collaboration, monitoring, and responsive coordination that adapts to changing needs and circumstances.
Support Coordination
Support Coordination is all about helping you understand the NDIS and your NDIS plan, linking you in with service providers you like, so you can reach your goals! Support Coordination is also about educating you about the NDIS and empowering you to make positive life choices. Your Support Coordinator is your NDIS expert who works with all your service providers and creates the necessary reports when your situation changes, or your NDIS Plan is being reviewed.
Psychosocial
Our Psychosocial Recovery Coaching service supports individuals living with psychosocial disabilities to build resilience, strengthen independence, and pursue a meaningful and fulfilling life. Recovery Coaches work collaboratively with participants using a recovery-oriented and person-centred approach to understand their strengths, goals, and unique recovery journey, helping them develop practical skills, increase confidence, and connect with relevant services and supports. We empower participants to take control of their lives, make informed decisions, and build the capacity needed to achieve their aspirations. This includes developing and implementing recovery-focused plans, strengthening daily living and self-management skills, building social connections and community participation, coordinating supports across health, mental health, and community services, enhancing confidence and wellbeing, providing support during periods of change or crisis, and promoting long-term independence and recovery outcomes through compassionate guidance and practical assistance.
Case Management
Our Case Managers will support you after your complex injury to access the services, treatments and supports you require to help facilitate transition from hospital to home, both during and after the rehabilitation phase. All our Case Managers are degree qualified health professionals who will work with you and your care team to build your capacity to manage your personal circumstances. We work closely with your funding provider (Insurance Commission of WA, Public Trustee or Private Trustee) to ensure all compliance is met and your needs are reviewed on a regular basis.
Social Work
Our Social Workers provide professional support to individuals, families, and communities facing personal, social, emotional, or environmental challenges. Using a strengths-based and person-centred approach, we help clients navigate life's complexities while promoting wellbeing, inclusion, and empowerment. We work collaboratively with each person to identify their strengths, address barriers, and access the resources needed to improve quality of life and achieve personal goals. This includes offering emotional support and counselling, advocacy and rights-based assistance, family and relationship support, crisis intervention and safety planning, community engagement and social inclusion, help accessing housing, health, education, and community resources, as well as support through life transitions and significant events. Our commitment is to help individuals and families build resilience, strengthen their support networks, and create positive pathways toward greater independence and wellbeing.
We Are Specialists In Supporting
We specialise in supporting individuals with a range of complex conditions including Acquired Brain Injury (ABI) — brain damage occurring after birth due to infection, disease, lack of oxygen or trauma; Psychosocial Disability (PSD) — arising from mental illness and affecting daily life, social inclusion, and wellbeing; Intellectual Disability — a lifelong condition affecting intellectual skills and behaviour; and Spinal Cord Injury — damage causing loss of function, feeling and mobility, often due to motor vehicle accidents, falls, sports injuries, or acts of violence.
